Registration Home

Registration Requirements

  • Lease/Mortage Agreement
  • Current Utility Bill
  • Birth Certificate
  • Immunizations (In Florida Form 680)
  • Physical (Must have been done within a year of registration)
  • Enrolling Parent ID
  • Any records from previous school. (Includes IEP, 504, Withdraw Form, Latest Report Card, etc.)

Registration Forms

If you want to complete a physical registration, you are able to use the documents below and bring them to the front office to get the student enrolled. You also have the option to stop by the school to get a physical copy of the registration and fill it out during your visit.

You are able to submit the registration in person by stopping at our school during our office hours OR you are able to email the registration along with the required supporting documents mentioned above to FRESRegistration@Osceolaschools.net. If you have any questions, please call our office at 407-933-9333.
